چکیده

Patients with bleeding oesophageal varices die from exsanguination or liver failure or a combination of these factors. The development of liver failure is inevitable if bleeding is allowed to continue, and the first aim of treatment must be to stop haemorrhage by the fastest and simplest method available. Unfortunately bleeding tends to recur unless the portal venous pressure can be permanently lowered ; but in practice the timing and choice of correct operation are difficult. Throughout the care of these patients medical and surgical therapy are closely linked, and the importance of combined consultation of physician and surgeon with formulation of a definite plan for each patient canxiot be overstressed.

Clinical and Paraclinical Findings in Children With Congenital Hepatic Fibrosis: A Single Center 10-Year Study

Background: Congenital hepatic fibrosis (CHF) is an autosomal hereditary disorder affecting the porto-biliary system. It is a rare hereditary disorder often presenting in childhood or adolescence with hepatomegaly, splenomegaly, and gastrointestinal bleeding. A timely diagnosis of organomegalies by sonography can prevent esophageal varices. Liver transplantation is now the only...

Therapeutic and prophylactic management of bleeding from oesophageal and gastric varices – recommendations of the Working Group of the National Consultant for Gastroenterology

Gastroesophageal varices are one of the most serious consequences of portal hypertension. One-third of patients with varices will develop variceal haemorrhage. Despite significant improvements in the outcomes of treatment, mortality due to bleeding from gastro-oesophageal varices still remains very high. These recommendations present optimal management of patients with non-bleeding and bleeding...

Oesophageal rupture in the course of conservative treatment of bleeding oesophageal varices.

Fatal oesophageal rupture is described as a complication of the management of bleeding oesophageal varices with repeated sclerotherapy and tamponade using the Sengstaken-Blakemore tube. The importance of chest radiographs is stressed in the early detection and prevention of malposition of the Sengstaken-Blakemore tube, as inflation of the gastric balloon in the oesophagus can result in oesophag...

Longterm outcome after injection sclerotherapy for oesophageal varices in children with extrahepatic portal hypertension.

A consecutive series of 36 children with bleeding from oesophageal varices secondary to extrahepatic portal hypertension was successfully treated by endoscopic injection sclerotherapy and followed up over a mean period of 8.7 years after variceal obliteration. There were no deaths from portal hypertension or its treatment and morbidity related to oesophageal sclerotherapy was minimal. Endoscopi...

LIVER DISEASE The role of the transjugular intrahepatic portosystemic stent shunt (TIPSS) in the management of bleeding gastric varices: clinical and haemodynamic correlations

Background: The transjugular intrahepatic portosystemic stent shunt (TIPSS) is effective in the management of both oesophageal and gastric variceal bleeding. Although it has been reported that gastric varices can bleed at pressures of <12 mm Hg, this phenomenon has been little studied in the clinical setting. Aims: To assess the efficacy of TIPSS on rebleeding and mortality following gastric an...
